A folha inclui campos (4) em que notas podem ser registradas e uma linha de enfraquecimento ou linha perfurada para destacamento (3, 5, 7) e, em uma configuração, os campos estão dispostos numa coluna de tal modo, que cada campo confina com o lombo (2).
