FICHA DE PLANEJAMENTO RADIOGRÁFICO E TOMOGRÁFICO. A presente Patente de Invenção refere-se a uma ficha impressa em folha transparente destinada a coletar e permitir o processamento de dados resultantes de uma radiograf ia associada à sondagem transgengival ou de uma tomografia bucal de modo a permitir a fabricação de guias cirúrgicas necessárias para a instalação de um implante dentário com a maior precisão. O presente invento é constituído basicamente de uma folha de transparência(1) contendo cinco campos impressos distintos de informações, quais sejam o campo de informações tomográficas (2), o campo de informações radiográficas(3), o relatório de atos vestíbulolingual(4), o relatório de atos mesio-distal(5) e o campo de informações do paciente(6).
