O envelope-caixa apresenta-se no formato retangular (1), limitado pelas linhas de dobra (2), dotado de serrilhado superior para abertura do envelope (3), orifícios o lacre (4), parte anexa destacável, compreendendo um cartão (5), vinco para dobra (6), parte colada (7), serrilhado para destacar o cartão (7.1), vinco lateral (8), e, após conformação, apresenta aspecto de uma caixa (9).
